Terms and Conditions – Standard QR Memorial Package

(Acrylic QR Code with YouTube Link)

By placing an order for our Standard QR Memorial Package, you agree to the following terms and conditions:

1. Service Description

This package includes the creation and delivery of a custom acrylic QR code plaque. The QR code links to a photo slideshow or video uploaded to YouTube, which serves as a digital tribute to your loved one.

2. Content Submission

• Clients must provide all photos, videos, and written content (e.g., names, dates, dedications) to be included in the slideshow or video.
  
  

• Please ensure all materials are final and high quality before submission. We are not liable for low-resolution or incomplete submissions.
  
  

3. YouTube Hosting

• The final video will be hosted on YouTube via our official channel. We may upload the video as Unlisted (only accessible via QR link) or Public, depending on your selection.
  
  

• You may request the video be taken down or replaced once, at no extra cost. Additional edits or re-uploads may incur service fees.
  
  

4. Design and Approval

• You will receive a preview of the video/slideshow and the QR plaque design for approval before production.
  
  

• One (1) round of revisions is included. Further changes may be subject to a fee.
  
  

5. Production and Delivery

• Acrylic QR plaques are produced upon final approval and are generally shipped within 5–7 business days.
  
  

• Delivery time may vary based on your location. We will provide tracking details upon dispatch.
  
  

6. Limitations

• QR codes link to external YouTube content. Eternal Tribute PH is not responsible for disruptions or limitations caused by YouTube's platform (e.g., availability, region restrictions).
  
  

• The physical QR plaque is water-resistant but not recommended for prolonged outdoor exposure.
  
  

7. Privacy

• By default, videos are in public,  unless if you request to make it private.

8. Refunds and Cancellations

• Cancellations can be made within 24 hours of order placement.
  
  

• After design approval or production, refunds are not available except for damaged items upon delivery (with valid photo proof submitted within 3 days).
  
  

9. Customer Responsibilities

• Please double-check the accuracy of all names, dates, and content before submission.
  
  

• You are responsible for monitoring any email or SMS notifications from us for updates, approvals, or clarifications.
